*Athlete Participation Requirements

As a reminder, all athletes must be up to date on their participation forms.  For initial eligibility, the form must be signed by a parent and doctor.  Every 3 years, the forms must be renewed prior to the expiration date.   To renew, you must first identify the status of the athlete.  “A” status requires parent signature only.  “AB” status requires both a doctor and parent signature.   If the form expires, then the athlete must complete the form with a parent signature and doctor signature again.  Please check the athlete list provided at http://www.soforsyth.com/registration.  Athletes not listed do not have an eligibility form on file or the form has expired (the athlete is unable to practice or compete without this form).  For questions, please contact Charlene Northcutt at cnorthcutt@forsyth.k12.ga.us.  Please note the posted list may not include forms submitted within 30 days of this email blast.  A copy of the participation form is attached to this email.  Once the form is completed, please fax to Charlene Northcutt at 770-888-1278.
