*Required Athlete Participation Forms

o    As a reminder, all athletes must be up to date on their participation/medical forms.

o   The list of athletes who are current on their forms can be found on the website.

o   For initial eligibility, the form must be signed by a parent and doctor.  Every 3 years, the forms must be renewed prior to the expiration date.   To renew, you must first identify the status of the athlete.  If the form expires, then the athlete must complete the form with a parent signature and doctor signature again (there are new state required forms this year so please be sure to use the 2016 forms).

o   Please check the athlete list provided on the Athlete Medical Expiration List website.  Athletes not listed do not have an eligibility form on file or the form has expired (the athlete is unable to practice or compete without this form).

o    For questions, please contact Charlene Northcutt at cnorthcutt@forsyth.k12.ga.us.  Please note the posted list may not include forms submitted within 30 days of this email blast.  A copy of the participation form is attached to this email.  Once the form is completed, please fax to Charlene Northcutt at 770-888-1278 or send directly to the SOGA office.
