Athlete Registration


Registration typically starts about 10-12 weeks before the games.

Check to see if your athlete is registered and up to date with his/her participation paperwork.

Medical Expiration List for Athletes and Families are listed by first initial and last name.

       The new medical form must be signed by the doctor every 3 years for ALL athletes.  If a form is expiring this year, athletes must use the new packet. Please be sure to check the athlete participation list for the expiration date.  If the athlete’s name is not listed or if the athlete is new, SOGA does not have a current medical form on file.   

  • *All forms will be mailed, emailed or faxed to the SOGA office (information is on the front page of the medical packet) and parents must keep a copy for their records (this is also a new procedure).  We will no longer use the old participation forms. If the parent of a school aged child decides to give it to the teacher or if a coach hands it in, we will fax/email it to SOGA for them.
  • The medical history form and release must be signed by parent/athlete every 3 years and submitted with the medical form signed by the doctor.

*Certain sports have a limited number of open spots due to the facility space, availability of coaches and schedules.  Athletes who competed in the state competitions last year, will have priority for their first choice sport.  Special Olympics Forsyth will make every effort to ensure registered athletes are able to compete in a sport.
