ATHLETE SPORT REGISTRATION

Special Olympics Forsyth County partners with the Forsyth County Therapeutic Program to help us manage the on-going Administration needs of our program.  We are grateful for this Partnership with the Forsyth County Parks and Rec Mgmt Team.

Medicals must be current for Fall Games no later than July 15, 2024 or you Athlete will not be able to participate.

A current Medical Form and Covid Waiver (forms below) must be on file for each Athlete as directed by SOGA.   Both are good for 3 years. 

If your Athlete’s name is not listed – your Athlete’s Medical is Expired.

REGISTER your Athlete:

1. Proceed to register your Athlete below by adding contact information. You will need their medical expiration date and 1^st and 2^nd sport choice.  
2. If your Athlete’s medical is expired enter “today’s date”.
   1. Contact your Dr’s office for an appt – or you can go to a CVS minute clinic under their “sports physical”.
   2. Download/obtain a copy of the form and bring it to your appointment to get it filled out completely with Dr signature.
   3. Email a copy to SOForsythforms@gmail.com – Athlete name in heading.  Please keep a copy of your completed form.
3. You can pick-up forms and/or drop off the signed forms at Central Park – Forsyth Therapeutic Program, to Jill Hite –no later than July 15, 2024 for Fall Games.  Medicals are good for 3 years.

A current medical must be on file for your Athlete to participate in any Special Olympics Forsyth County activity.
